The Pearl Nursing Center of Rochester

Choosing between the many Rochester, NY nursing homes for you or a loved one can seem a bit overwhelming. The Pearl Nursing Center of Rochester* has created a user-friendly admissions process through an on-site Social Worker and streamlined forms. All accepted are Medicaid, Medicare, long-term care, and private insurance.

Admission steps to complete are different for those entering The Pearl* from home or from the hospital. Coming from home, the process may take up to a week. The completed documents needed include:

• Patient Review Instrument (PRI) (completed by a certified professional)
• Screen (completed by a certified professional)
• Admissions Application

If your loved one is in the hospital, please inform your social worker that you are interested in our independently operated nursing home, and would like a referral to be sent to The Pearl*. The social worker will take care of sending the needed information. The Director of Social Work/Admissions Coordinator at The Pearl* will review the information with the Director of Nursing and Administrator. A decision will be made within 24-48 hours. At the point of paperwork submission, we encourage family members and friends to schedule a tour of The Pearl*.

Upon Admission, a new resident chooses between a spacious private room or a companion roommate. With a high staff-to-resident ratio, our well tenured staff delivers their commitment to the on-going care of your loved one. Compassion and professionalism are shown through The Pearl Nursing Center of Rochester’s* nurses, dietary specialists, one-on-one physical, occupational, speech, and music therapists, social, physical, and intellectual activities personnel, social worker, and nursing assistants. Pearl* caregivers will grow to know your loved one and family on a personal level. We are staffed to spend the extra time it takes to understand and focus on the resident’s unique needs and goals. Our staff, along with the resident’s physician and family, builds a strong support system. This team creates and maintains a custom-tailored plan of care for your Pearl* family member.
