In addition to our healthcare services, Sugar Creek Care Center also offers a limited number of independent living apartments on-site, providing residents with the benefits of proximity to skilled nursing and rehabilitation while maintaining their independence.

Our facility is dedicated to meeting the diverse needs of our patients, offering various programs tailored to those who require 24-hour services but no longer need hospital care. At Sugar Creek Care Center, we are committed to ensuring the best possible quality of life for our residents and their families.

Admission Information

Understanding that transitions can pose challenges for both you and your family, we believe that knowledge is key to easing the process for everyone involved.

We invite prospective residents and their families to arrange a tour and meet with our experienced staff. This step ensures a seamless transition from the hospital or home environment to Avalon Care Center. Our dedicated Admissions Coordinator will expertly navigate you through the admission and transfer procedures.

Furthermore, our Community Liaisons collaborate closely with patients, families, hospital staff, and physicians to discern the most suitable treatment and care environment. This personalized approach ensures that each individual’s needs are met with the utmost attention and care.
