This step ensures a seamless transition from the hospital or home environment to Avalon Care Center. Our dedicated Admissions Coordinator will expertly navigate you through the admission and transfer procedures.

Furthermore, our Community Liaisons collaborate closely with patients, families, hospital staff, and physicians to discern the most suitable treatment and care environment. This personalized approach ensures that each individual’s needs are met with the utmost attention and care.
