WeCare at The Green Home offers skilled nursing and rehabilitation services for residents who no longer require hospital care but need 24-hour skilled support. Our dedicated team works with each resident to help improve quality of life, support independence, and address physical, psychological, and social needs.

Our rehabilitation team includes physical, occupational, and speech therapists, with care overseen by a board-certified medical director in partnership with each resident’s physician

Comprehensive Skilled Nursing Care

The Green Home provides 24-hour skilled nursing care for residents who no longer require hospital care but still need ongoing medical support, supervision, and assistance with daily needs. Caring, dedicated staff provide services that address each resident’s physical, psychological, and social well-being, with care overseen by a board-certified medical director in partnership with the resident’s physicians.

Rehabilitation

The Green Home’s rehabilitation program helps residents rebuild strength, coordination, and independence after an illness, injury, surgery, or hospital stay. The rehabilitation gym offers equipment designed to support recovery, while physical, occupational, and speech therapists work with residents to create a personalized care plan focused on helping them return to their highest level of functioning.
